Form Submissions
On sites with forms enabled, the responses people submit flow into the portal so you can review them in one place. This page explains where managed-form responses land, how to work through them, and how sensitive forms are handled differently.
Managed forms are authored in the Form Manager inside the page editor and rendered on your live site by the shared DCS form runtime. When a visitor submits one, the response is posted to the platform and stored against your site.
Where responses go
There are two related inboxes:
- Form responses — intake and questionnaire forms appear here, one form at a time. Pick a form from the selector to review its responses in reverse-chronological order.
- Contact inbox — standard contact forms appear in the Contact inbox instead, so general inquiries stay separate from structured intake.
Working through responses
On the Form responses screen you can:
- Search across a response's submitter name, email, and answer content.
- Filter to flagged (PHI) responses — narrow the list to submissions that were flagged for personal health information.
- Open a response to see its full detail.
- Delete a response permanently — used to satisfy right-to-delete requests. Deletion is permanent.
Each submission records details that support later review and compliance audits, including the form's version at the time it was submitted, a timestamp, and the submitter's IP address.
Sensitive forms
A form can be marked sensitive (for example, a law-firm intake or a health-related questionnaire). Sensitive forms carry platform-enforced protections that authors cannot override:
- Notification emails suppress the submission content. For a sensitive form, the "new submission" email a recipient receives contains only a link back to the portal — never the field values, names, or any of the submission body. You review the actual answers securely in the portal.
This keeps confidential intake out of email while still letting you know a response has arrived.
